Adjustment, Deductible & Co-pay Reconciliation: Apply the appropriate write-off codes to maintain patient ledger balance; confirm contractual fee schedule adjustments, coinsurance, co-pays, and deductible amounts in accordance with payer contracts. Denial Code Categorisation and AR Routing: Recognise line-item rejections, partial payments, zero-pay remits, and claim denials; correctly assign reason/remark codes (CARC/RARC) and direct flagged claims to A/R specialist queues for prompt appeal. Daily Batch Balancing & Deposit Reconciliation: To guarantee 100% financial balancing, reconcile daily posted payment batches against bank deposit sheets, lockbox statements, clearinghouse ERA reports, and merchant processing logs.
